This form creates a downloadable Excel spreadsheet, for use by the selected discipline in tracking SLO assessment results for the selected semester.
Choose a semester from the menu above, and then choose a discipline from either the subject code menu or the discipline menu. Then a link will appear, for downloading the spreadsheet. Save it to your computer, open it in Excel, and format it and use it as needed to enter, track, and evaluate course SLO assessment results for the semester.
Note that this form is to be kept by the department for documentation and evidence associated with the SLO assessment process. It is not meant to be shared outside of the department -- not with the Reseach Office nor with the Instruction Office -- not with the Instructional SLO Facilitator. But departments can use the form to gather and consider the results as they decide what to enter into the eSLOs database for their assessed courses.
